Most dental plans include limitations such as yearly maximum benefits, frequency restrictions (for example, one X-ray every three years), or a percentage-based coverage limit (such as 80%). Certain procedures may not be eligible for coverage or may only be partially reimbursed. In many cases, plans cover standard dental fees but not specialist fees.
If your plan does not fully cover a procedure, any uncovered portion is the responsibility of the patient (or guardian). Payment for uncovered amounts is due on the day of treatment, even if your plan states 100% coverage.
